Please answer the following queries below and indicate how the training you have received has met (or exceeded) the requirements for IBTA Certified Practitioner Standards as outlined.

You will need to submit this form in addition to your Practitioner Application.

In the absence of some of the equivalency requirements the membership review committee may ask for references from an IBTA Certified Practitioner or other supplementary documentation following the examination of your submission.
